General Course Requirements and Recommendations
  1. Students must be present for all scheduled clinical rotations. Missed days must be made up before the end of the rotation.
  2. The clinical site and the program director must be notified at least a half hour before the rotation begins if the student misses a rotation.
  3. Attendance logs will be completed at the start and end of each day the student attends the site. The supervisor will initial the log time for each entry.
  4. Clinical Logs are to be completed by each student at the end of each clinical day to provide a basis for evaluating their competency and progress in the clinical rotation. The log will be turned in to the site supervisor at the end of each week for review and comment on the student's progress. The supervisor will meet with each student at the end of the week. The weekly logs will be mailed or faxed to the program director every two weeks.
  5. The performance evaluation reports: Affective Domain and Clinical Performance Evaluation, will be shared with the students at the end of the rotation. Comments are recommended as feedback for the student and site supervisor. All reports are retrieved by the program director or clinical assistant at the end of the student’s rotation during the Training Experience Review.
  6. Picture identification is required for admittance to the pharmacy externship site.
  7. The clinical site dress code must be followed according to the Site Information Form. Students must comply with dress code to complete rotation successfully.
  8. Students must pass each externship rotation site with a grade “C” or better.
